Medical Requirements

Arizona requires all CDL and CLP holders to maintain a current Medical Examiner's Certificate (DOT physical) on file with MVD. The examination must be performed by an FMCSA-listed medical examiner. Medical certificates are typically valid for up to 24 months but may be issued for shorter periods based on medical conditions. All CDL/CLP drivers, including military personnel, must keep current medical certification on file. If the medical certificate expires or is not maintained, the CDL may be downgraded. The DOT physical examines vision, hearing, blood pressure, cardiovascular health, diabetes control, seizure history, and substance use according to FMCSA standards.
